Responsibilities Under the general direction of the Case Manager, assess social and emotional factors related to the patient’s illness and nursing requirements in the home. Develop and implement a plan for home social work services, including education for patients, family members, and caregivers. Coordinate with other home health staff and community resources to integrate care efficiently. Assist patients and families/caregivers with education and support to cope with recent diagnoses. Qualifications Masters Degree from an program accredited by the Council on Social Work Education required. Basic Life Support (BLS) certification required. Driver license and automobile insurance required. LCSW license highly preferred (supervision provided to license-eligible Social Workers). One year of MSW experience in a healthcare setting strongly preferred. Knowledge of local community resources; knowledge of state and federal homecare regulatory guidelines preferred. Excellent written and verbal communication; ability to work with diverse groups and maintain professional relationships. Ability to manage time, handle sensitive issues, and operate in a dynamic home health environment.
