What you'll do:

• Develop relationships and collaborate with primary care tea to conduct social services assessments to determine the appropriate needs for each member.
• Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
• Navigate managed care plans for community services and programs.

Requirements:

• Experience working with patients in need of social services.
• Experience with assessments and planning for social services.
• Working knowledge of different diagnosis of common health conditions.

Education and Experience:

• Fully licensed LMSW in NE.
• Master's degree in Social Work.
• 1+ years' experience in clinical social work
• Fluency in Spanish or other language spoken by people in the communities we service.
