Low-Income  Aging Care Manager

Your Elder Experts at Jewish Family & Children’s Services has grown into the largest not-for-profit Aging Life Care program in the Greater Boston Area. Our talented and caring professionals guide older adults and their families through the complexities and challenges of aging well. We have expertise in working with people with dementia, Parkinson's Disease, ALS, and other chronic illnesses, though we work with people in all situations. Whether through one-time consultation or ongoing case management, we help our clients in myriad ways: exploring housing options, understanding their benefits and insurance, learning about new programs and resources, communicating effectively with family members around difficult issues, and coordinating medical services, home care, and community supports. We also accompany our clients to medical appointments to provide accurate history, communicate current issues and make sure that treatment plans are followed-up on. We are experts who care.

 

 

About the Role: 

Based on a thorough assessment, the Low-Income Care Manager will work to identity and prioritize needs and connect individuals with resources including safe and affordable housing, benefits, homecare, medical care, legal and financial support. The Care Manager will provide hands-on direct assistance and case management to clients as needed to achieve their goals, while providing expert support through difficult transitions due to medical, financial and housing crises. 

Essential Job Functions:

  • Develop and implement personalized care plans based on needs assessment and client goals
  • Connect and coordinate client to critical services in the community including affordable and sustainable housing (independent and supported)
  • Serve as liaison to other JF&CS services including legal, financial, and basic needs programs
  • Guide and support clients in accessing and coordination of medical care
  • Accompany and transport clients to critical appointments
  • Collaborate with family and caregivers both locally and at a distance
  • Participate in team meetings, individual and peer supervision, and maintain online case record documentation
  • Other duties as assigned 

Education/Experience Requirements: 

  • Master’s degree in social work/related field or 5+ years’ experience providing similar functions 
  • 2-4 years’ experience working with older adults
  • Experience/knowledge of dementia and mental health care
  • Knowledge of community resources, affordable and subsidized housing and community benefits
  • Access/use of personal vehicle and valid driver’s license required.
  • Proficient with the use of Microsoft Office Suite (Word, Outlook, Power Point) and willingness to learn and use technology for documentation and communication
