Job Description

As a Nurse Care Manager, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.

Key Responsibilities:

• Collaborate with members and their families to maintain the most independent living situation possible.
• Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.
• Authorize covered services and coordinate care across different payers.
• Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.

Qualifications:

• Current NYS RN License and registration
• Associate's Degree in Nursing Required. Bachelor of Science in Nursing (BSN) Preferred
• Minimum of 3 years of RN experience
• Clinical expertise in geriatrics preferred; managed long-term care experience required
• Bilingual Chinese,Cantonese or Mandarin required

Compensation:

• Annual salary: $105,000 - $115,000
• Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)
