What You’ll Do :

• Provide home health visits, admissions, and assessments for skilled nursing care: wound care, med administration, disease management
• Develop and follow individualized care plans in collaboration with physicians
• Monitor and document patient conditions via electronic tablet charting
• Serve as case manager and care coordinator for your clients
• Build meaningful relationships with clients and their families

What You’ll Need :

• Active Massachusetts RN license
• A passion for home health care and helping people live independently
