Key Responsibilities Medication Management Support: Provide guidance on safe prescribing, administration, and storage of medications within the hospice. Support the implementation of best practices in medicines management in line with national guidance and regulatory requirements. Clinical Governance & Compliance: Conduct regular medication audits to ensure compliance with policies and relevant healthcare regulations. Provide recommendations on the safe handling, storage, and disposal of medicines, including controlled drugs where applicable. Ensure all policies related to medication management are up to date and reflect current best practices. Support medication incident investigations and oversight of any actions/learning. Education & Training: Deliver training sessions to nursing staff and healthcare assistants on medication management, administration, and updates in prescribing guidelines. Offer guidance on key palliative care medication topics, including symptom management and drug interactions. Act as a point of contact for medication-related queries from hospice staff. Liaise with local GPs, community pharmacies, and specialist palliative care teams to support seamless medication provision. Support hospice staff in understanding and addressing medication-related issues in patient care. Stock & Supply Support: Advise on the management of stock medications (paracetamol and adrenaline) and recommend additional essential stock items. Work with local pharmacies to facilitate timely access to prescribed medications for patients. General Duties Work collaboratively with colleagues at Beaumond House. Respect confidentiality in all areas of work. Abide by the Health and Safety at Work Act. Work within Beaumond House policies and procedures. Participate in and contribute to team meetings. Maintain professionalism, reflecting Beaumond House values. Skills, Experience, and Attributes Registered pharmacist with the General Pharmaceutical Council (GPhC). Experience in palliative care, hospice, or community pharmacy settings. Strong understanding of medication management in a nurse-led environment. Ability to deliver training and provide expert guidance to healthcare professionals. Excellent communication and collaboration skills. Prior experience supporting hospices or care homes. Understanding of transcribing procedures and community-based prescribing. Knowledge of policies and procedures in medication governance.
